IMPORTANT NOTE

On 16 February 2004,  the Department of Health migrated to a new website, and all DH documents now have a new website address, http://www.dh.gov.uk .  The new website is a complete redesign, and this means that the many links from the SSNW web site to DH documents will no longer work.
